Avita Community Partners is a non-profit organization based in Cleveland, Ohio that provides services to people with developmental disabilities, mental illness, and addictive diseases. Our mission is to promote safe, stable, and meaningful lives for citizens within a 13-county area of northeast Georgia. We offer a broad range of services and programs to help individuals, families, and communities to minimize barriers and achieve their dreams. We strive to build strong communities that support all citizens by providing comprehensive services and support to those in crisis. Our vision is for individuals we serve to participate in a variety of relationships, have a home, job, and meaningful interaction in the community, and for families of these individuals, biological and chosen, to serve as valued partners who share knowledge and expertise and participate in planning.
