About Child and Family Connections #12 at Easterseals

At Child and Family Connections #12 at Easterseals, our team is devoted to providing quality, individualized care for infants, toddlers, and their families. Our staff is proactive and collaborative, forming strong partnerships within the community and offering a comprehensive set of services. From processing referrals and determining eligibility for early intervention services to developing an Individualized Family Service Plan (IFSP), monitoring ongoing IFSP services, providing transition services, conducting Child Find activities, and promoting public awareness, we strive to ensure that every family enrolled in our early intervention system receives the support they need.
