Our Person-Centered Planning approach focuses on ensuring that the services people receive support their choices in where they want to live, how and with whom they choose to spend the day, and their hopes and dreams for the future. By providing services and linking people to other helpful and necessary resources in the community, SDRC is the central point for families to access the support they need.
